Hamilton Middle School will be administering the GameChanger Prevention Survey with our students on 10/17/25. The survey asks about school climate, stress, and behaviors related to substance use. The results will help schools and the GameChanger team improve programs that support student health and well-being. The survey takes about 10-12 minutes for students to complete. All responses are anonymous. If you do not want your child to participate, please contact the school or return the slip that will be mailed to your home by 10/16/25. Thank you in advance for your consideration.

We ask that if you have not already done so, please review this compact with your child, and fill out the form. This is required through our new Title I funding.
